Addiction and Mental Health - Opioid Dependency Program

Opioid Agonist Treatment (OAT) medications (ie. methadone, Suboxone®) are available in all locations. Most OAT medications allows a person to function normally without drowsiness or withdrawal symptioms. In cases where OAT medications are not effective hydromorphone can be accessed in Narcotic Transition Services (NTS) locations (Grande Prairie, Edmonton, Red Deer, Calgary, Medicine Hat, Lethbridge). In all locations, individuals have access to medical support, medication and psychosocial support (housing, financial, mental health, legal, relationships).

Services available in all clinics may include:

• ongoing support and monitoring (e.g. prescription management, medical follow up, urine testing, counselling and treatment planning)
• health, safety and risk reduction teaching and management
• source of information and expertise for community stakeholders on Opioid Agonist Treatment and the medications used
• links to community pharmacists
• links to other addictions treatment services
• links to other social and health support systems
• hepatitis and HIV screening

Adult Neurorehabilitation Clinic - Pediatric Onset Disorders

Adults with pediatric-onset disorders such as cerebral palsy, spina bifida and genetic syndromes impacting the neurologic or muscular systems may have ongoing concerns related to movement and daily function. As specialists in physiatry (also known as physical medicine and rehabilitation), we provide assessment and treatment for these impairments.

AHS ID Program

Offers support to clients who are serviced by AHS programs and services, both in acute care and community;

• client assistance to obtain IDs
• storage solutions for clients requiring secure storage of newly-acquired IDs until they are permanently housed

Alberta Healthy Living Program - Calgary Zone

The Alberta Healthy Living Program (AHLP) offers health education classes, self-management workshops, nutrition counselling and supervised exercise for adults living with chronic (long-term) conditions. All services are offered by Zoom, by phone, and / or in person.
Health Education:
Adults 18 years of age and older can learn how to manage chronic condition(s) or disease(s) with our health education classes. Some class topics include:

• diabetes, heart disease, osteoarthritis, obesity, chronic pain, COPD, celiac disease, osteoporosis, fatty liver disease, Parkinson disease, stroke, and more
• weight management, healthy sleep habits, meal planning, stress, grief, better breathing, and more
• grocery store tours and cooking demonstrations
• how to manage long-COVID symptoms

Self-Management Workshops:

Supervised Exercise Program:
Do you have questions about how exercise can help you manage your chronic health condition? The Supervised Exercise Program will help you better understand the positive impact of exercise on managing your condition(s) and how to begin or modify an exercise program based on your symptoms and abilities. A healthcare professional will guide you through the program and help you develop strategies to overcome barriers to being physically active. You will be provided with opportunities to attend educational sessions, exercise in a supervised group setting, participate in health-coaching, and work on an action plan to continue exercising at home or in the community. In addition to our online offerings, some of our group exercise classes are offered in-person out of several locations around the City of Calgary. All participants must have at least one chronic condition and live in Calgary Zone to be eligible for these services. Nutrition Counselling:

Adults living with a nutrition-related health condition are eligible for nutrition counselling with a registered dietitian. Participants will be asked to attend a relevant health education class, if available, before booking a nutrition counselling appointment. Nutrition counselling appointments can be conducted either over the phone or in person.

Assertive Community Treatment - Addiction and Mental Health

In the Calgary Zone, ACT (Active Community Treatment) operates out of the Sheldon M. Chumir Health Centre. They provide support during business hours, Monday to Friday.

The teams provide comprehensive services including outreach to adults with severe and persistent mental illness who are struggling to engage in less intensive addiction and mental health outpatient or community services. ACT offers a strengths-based and holistic approach to promote optimal wellness and recovery to people with challenges related to a primary diagnosis of schizophrenia, schizoaffective disorder, bipolar disorder, and other psychotic disorders who may have concurrent substance use. ACT advocates and supports access to individualized care.

Services offered by a multidisciplinary team in a person's community and may include:

• harm reduction approach to care
• psychiatry
• liaison with family and community services
• support with medication management and CTO monitoring
• advocating and connection to housing, medical needs, legal and financial services
• access to psychosocial, recreational programming, psycho-educational, and peer support services

Behavioural Education and Support Team

A group of mental health clinicians and educators provide support to staff and clients in long term care and supportive living facilities to help manage mental health concerns and complex behaviours.

Support includes:

• clarifying what the client needs and the reason for challenging behaviors and to build capacity in site staff to manage complex clients
• collaborative assessment of client with site care team
• joint development of care plans
• supporting the use of strategies
• education to staff about mental health generally, caring for clients with mental health concerns, behaviour management

The Behavioural Education and Support Team (BES) works in tandem with the Geriatric Mental Health Consulting Team.

Offers fresh food that people can buy at AHS hospitals and facilities. Vending machines are also available.

Calgary Ambulatory Lymphedema Service

The Calgary Ambulatory Lymphedema Service (CALS) provides assessment and treatment for people with primary or secondary (non-cancer related) lymphedema to reduce and maintain edema and promote self-management of lymphedema. CALS is a non-urgent program.

Service may include:

• group education session
• comprehensive assessment
• skin care treatment
• compression bandaging
• exercise prescription
• teaching on self-management (bandaging / manual lymphedema drainage)
• fitting for compression garments
• completion of Alberta Aids to Daily Living (AADL) authorization for day-time compression garments
• education to promote self-management of lymphedema at home

Calgary Sexual Assault Response Team

CSART is a team of registered nurses, doctors and crisis counsellors who have specialized training and expertise to support people who have been sexually assaulted.

If you or your child has been sexually assaulted in the last 7 days ( 168 hours ), we can offer confidential services including:

1. medical treatment with no police involvement or
2. medical treatment with police involvement, forensic exam and evidence collection or
3. medical treatment, forensic exam, evidence collection and storage. You can have up to 1 year to decide if you want to report to the police

A crisis counsellor from Calgary Communities Against Sexual Abuse ( CCASA ) can provide emotional support and strategies for coping after a sexual assault, assess your immediate needs, and follow up with you if you choose.

Cardiology - Electrocardiogram Services

Provides ECGs to diagnose possible heart conditions. Cardiologists and the Internal Medicine department reviews the results and sends a report to the referring / ordering doctor.

Cashier Services

Cashier services performs financial transactions, including the acceptance of payment for patient fees.

Communicable Disease Control

Under the authority of the Medical Officer of Health, the program is responsible to prevent and control communicable diseases (CD) which are notifiable (under the Communicable Disease Regulations of the Public Health Act of Alberta). Strategies to do this include:

• preventing infection through teaching and prophylaxis (including immunization)
• limiting the spread of infections by timely investigation and follow-up of notifiable CD incidents and putting protective measures in place
• providing epidemiologic data for reporting and public health response measures
• continuous monitoring and surveillance of CDs in the community so they may be found and responded to before people get sick or there is a disease outbreak

Community Accessible Rehabilitation (CAR) - Musculoskeletal Rehabilitation

Treatment offered for complex musculoskeletal conditions that may include:

• hand and upper extremity (arm) injuries, including specialized custom hand splint fabrication
• multiple, complex injuries due to trauma
• amputation
• certain rheumatology diagnoses
• complex joint arthroplasty

The rehabilitation focus is on achieving client goals in improved functionality and independence, to enable return to work or meaningful activity. This will include one-on-one interventions, education, exercise, self-management strategies, and where appropriate, connection with other community resources.

Community Accessible Rehabilitation (CAR) - Neurological Rehabilitation

Short term rehabilitation program for adults who recently experienced a neurological event or who have a neurological condition, who would benefit from an active rehabilitation program with an interdisciplinary team. Rehabilitation focus is to support clients in addressing their goals to improve function and independence or return to meaningful activity / roles. This may include:

• one-on-one and/or group interventions
• education
• exercise
• self-management strategies
• where appropriate, connection with community resources

Community Accessible Rehabilitation (CAR) - Pre-Driving Assessment

The clinic offers assessment of psychomotor skills, visual perceptual ability, cognition, general awareness, and reaction speed to clients who have a disability as a result of a neurological injury. Based on the results of the assessment, recommendations regarding return to driving are made to Alberta Driver Fitness and Monitoring Branch. They will review the recommendations and make the final decision on the client's fitness to drive.

Community Addiction and Mental Health Clinics

Services may include:

• intake assessment
• psychiatric consultation
• individual and or group therapy

• transition services (e.g. Independent Living Support)

Community Extension Team - Addiction & Mental Health

CET is a mental health transition team that works with individuals who have traditionally not connected to Mental Health follow-up due to a multitude of barriers.

• referrals come from Hospital (typically inpatient Psychiatric Units). No community referrals accepted
• need to have a diagnosis of a Primary psychotic disorder or Mood disorder with a psychotic component
• CET will meet with them on the Unit prior to discharge and provide a bridge to a longer term mental health clinic
• CET provides an intensive outreach support that works with individuals on a Community Treatment Order and manages medication
• access to Psychiatry to manage medication while awaiting connection to Mental Health Clinic

Community Geriatric Mental Health Service

Services provided may include:

• assessment, treatment and follow-up
• caregiver support
• one-to-one and group treatment for seniors with mental health concerns (e.g. anxiety, depression, schizophrenia and other psychiatric disorders)
• clinic and in-home service
• education for the client, caregiver, and community partners

The family doctor and other caregivers are involved in the assessment and treatment plan.

This service does not offer crisis intervention.

Day treatment Services: Group therapy, for individuals experiencing anxiety and depression, psychiatry consultation also available.

SUILL: Three phase group based therapy for individuals seeking group work for substance use. Psychiatry consultation also available.

Community Geriatric Mental Health Service - Psychiatric Consultation

Accessing this service has to specify request for psychiatric consultation when seeking one or more of the following:

• diagnositc clarification
• medication review
• recommendaiton for treatment / mangement of psychiatric symptoms

Referral may be considerd for pesons age 65-69 if requiring psychiatric consultation and also are challenged with age related (geriatric) frailty or diseases (ie: dementia, advanced parkinsons, advanced cardio vascular disease)

Computed Tomography

Computed tomography, also known as "CT Scan" or "CAT Scan", is when pictures of the body are taken using radiation and computer-enhanced imaging.

Conservative Management Care - Alberta Kidney Care - South

The conservative management program is for patients who decide they would prefer not to include dialysis as a part of their treatment plan. The program utilizes a multidisciplinary approach to:

• prevent and treat any symptoms or complications of end stage chronic kidney disease
• preserve current kidney function and delay further progression to end stage kidney disease
• provide supportive holistic care when needed
• help the patient and family members plan for the future and end of life
• increase access to supportive care in the community

Diabetes Centre - Endocrinology & Metabolism Program

Services provided:

• diabetes education / counselling with nurses, dietitians, or a pharmacist (topics include an overview of the disease, complications, medications, exercise, and nutrition)
• medication adjustment
• insulin pump therapy
• for information on the Alberta Insulin Pump Therapy Program (IPTP), visit the Diabetes Insulin Pump Therapy website
• pregnant with diabetes: refer to Diabetes in Pregnancy Clinic (DIP) at the hospital in which they’ll deliver
• referral to Diabetes Centre Calgary serves as indication to enact the Provincial Diabetes Medication Adjustment Protocol (HCS-296-01)

Dialectical Behavioural Therapy - Addiction & Mental Health

The DBT (Dialectical Behavioral Therapy) Program provides evidence-based treatment to individuals diagnosed with borderline personality disorder living in the community who engage in high-risk behaviours and/or have experienced poor outcomes and/or engagement in community clinics or DBT informed treatments. Treatment is intensive and comprehensive, focusing on goals and based on treating behaviours according to a target hierarchy, prioritizing safety interfering behaviours followed by therapy interfering behaviours. Services include support around eliminating high risk behaviours such as self-harm, suicide, substance use, disordered eating, etc.; therapy to help individuals develop effective coping skills to build a life worth living; and treatment of PTSD symptoms when indicated. The DBT Program is time limited, generally 15 to 18 months in length. To participate in this program, clients are required to:

• attend weekly individual therapy
• attend weekly DBT Skills Group
• utilize between session skills coaching
• complete weekly homework
• complete daily diary cards
• work towards solving problems in a way that does not include intentional self-harm and / or suicide

These transitional units were developed to support clients from acute care in their rehabilitation prior to their discharge to the most appropriate living situation.

Offers services for people who have glomerulonephritis (GN), a type of kidney disease caused by an autoimmune disease to:

• improve early detection and treatment
• reduce and minimize frequency and severity of hospitalizations
• delay progress and the need for renal replacement therapy
• provide comprehensive care plans

Hemodialysis - Alberta Kidney Care - South

Provides hemodialysis treatments (a process where the client's blood is passed through a special filter that removes waste products and water). Most people need 3 treatments every week. Each treatment lasts 4 hours.

Home Hemodialysis - Alberta Kidney Care - South

Provides training, supplies and equipment to clients to help them do hemodialysis in their own homes.

Many different dialysis schedules are available. Hemodialysis can be done during the day or at night while the client sleeps.

The clinic is led by a nurse practitioner who works in close collaboration with a nephrologist. Specifically, the role of the nurse practitioner will include providing independent assessments, diagnosis, management, evaluation and follow-up for patients who have diabetes, difficult to control hypertension and / or dyslipidemia. Difficult cases will be managed by consultation with a nephrologist.

Sexually Transmitted Infection (STI) Clinic

STI Clinic Process:

• accepts walk-ins only
• due to high demand, please expect to wait to be seen
• urgent concerns are prioritized, which include:
  - patients with a known STI and require treatment
  - contact to a known STI and require testing/treatment
  - symptoms of an STI
  - care following a recent sexual assault
• routine testing is provided
• self-collection testing is available
• all clients are required to perform hand hygiene and wear a mask at all times, new masks will be provided onsite – personal masks are not permitted

STI Clinic services provided:

• diagnosis and treatment of sexually transmitted infections in a non-judgmental and inclusive environment
• detailed syphilis diagnosis, treatment and follow up
• Hepatitis A, B and HPV vaccine program for qualified individuals
• HIV, hepatitis A, B and C testing for qualified individuals
• post sexual assault care (excluding forensic examinations)
• emergency contraception
• HIV PEP/nPEP referrals
• pap smears and follow up for those unable to access care elsewhere
• accepting referrals to infectious disease physicians who specialize in STI and HIV care
• counselling and client education as well as support for community partners and physicians
• contact tracing and partner notification
• testing and treatment provided in community to those unable to access other sites or providers
• harm reduction supply distribution (e.g. needles, condoms)
• community based naloxone kits and training
• referrals to other services

Sheldon Chumir Immunization Clinic

The Sheldon Chumir Immunization Clinic offers immunizations to select groups only:

• transplant clients (referral required)
• clients with bleeding disorders (referral required)
• individuals living with chronic kidney disease (referral required)
• individuals who elect to follow a non-routine immunization schedule for their child
• clients anticipating or on immunosuppressive medication or treatment (referral may be required)
• clients with asplenia (or anticipating asplenia) and cochlear implants
• adults requiring routine immunization services
• students attending an alternative learning environment
• clients from Elbow River Healing Lodge

Clients who are not eligible and should seek services at their local community health centre via HealthLink 811:

• children aged 0 - 6 following routine vaccine schedules
• school-aged children 7 and up not attending an alternative learning environment

Southern Alberta Clinic

Provides medical care to all HIV - positive patients in southern Alberta. This program provides:

• intake appointment - with a nurse and social worker to provide introduction to the clinic and education about HIV
• first medical visit - medical history and assessment; test results are reviewed and a management plan is made
• follow-up appointments - usually about 1 month after the first visit and then every 3- 6 months afterwards
• nursing services - coordinates care within the clinic and with partner agencies, provides HIV health education
• social work services - service navigation and resources, counselling
• pharmacy - provides medication counselling and dispenses HIV medications

STI Physician Clinic

The STI Physician Clinic provides complex care for the following patients:

• Neurosyphilis assessments
• Lumbar puncture tests for patients with suspected neurosyphilis
• Complex management for herpes simplex virus (HSV) and human papilloma virus (HPV)
• Unresolved and persistent STI symptoms

Stroke & Brain Injury Early Supported Discharge Team

To facilitate early supported discharge of stroke or brain injury survivors with mild or moderate disability by providing rehabilitation services to enhance transition back into the community.

• ESD (Early Supported Discharge) accepts referrals for clients living in the city of Calgary, and extending 25 km beyond city limits. Referrals are accepted from all acute and tertiary rehabilitation sites
• Length of service (LOS): Stroke - 1 to 8 weeks; Brain injury - 1 to 12 weeks
• Clients will be provided with short term, client driven, community / home based rehabilitation interventions
• up to 3 hours per day of treatment based on client need
• up to 5 days / week daily therapy based on client need
• single discipline referrals accepted

Supervised Consumption Service

Includes:

• medical interventions by regulated health care providers in the event of opioid / stimulant overdoses, including oxygen and naloxone administration
• sterile injection supplies and disposal equipment
• drug poisoning prevention education and response training, including naloxone kits
• education on a variety of harm reduction topics, such as vein care, safer injection techniques, and hepatitis / human immunodeficiency virus (HIV) prevention
• connection to recovery-oriented services such as substance use treatment and supports, mental health care, and housing

Access to the following only for those using the site:

• client advocacy and referrals to health, social and recovery-oriented services
• basic wound care and health services (will vary by site)

This service is provided by Alberta Health Services and Alberta Health Services contracted service providers.

Switchboard

These services include:

• answering incoming telephone calls
• transferring telephone calls and
• giving out authorized telephone numbers

Tuberculosis Testing

Offers free tuberculosis (TB) skin testing to eligible Albertans. The Mantoux skin test is not recommended to diagnose TB.

Testing is done at public health centres throughout the province:

• North Zone
• Edmonton Zone
• Central Zone
• Calgary Zone
• South Zone

If you are not eligible for free testing but still need to be tested there will be a charge for the test.

For travel or work related TB testing contact the Travel Health Services in your area.

Urgent Care Services

Urgent care services are for people who have unexpected but non-life-threatening health concerns that usually require same-day treatment. Some examples include broken bones, sprains, lacerations, asthma, dehydration, pain and infections.

All patients are seen according to the urgency of their health condition, with the sickest patients being seen first.

Urgent Mental Health

Urgent mental health services are available with no appointment through the Urgent Care department. Individuals need to go through triage and register with Urgent Care. Please note there is no psychiatrist on call and clinicians are not able to make medication changes.

Urgent Mental Health conditions may include:

• depression
• anxiety
• thoughts of suicide or harming oneself
• overwhelming stress
• addictions
• other situations that cause a person to be in crisis

Hours and services may vary by location. Please select the location you are seeking to determine hours and whether both mental health assessment and single session therapy are offered.

Volunteer Resources

Our volunteer roles share a commitment to improving the quality of the patient and family experience.

Volunteers are:

• way-finders who help guide patients and families around our busy facilities,
• friendly visitors who spend time with our socially isolated patients,
• patient and family advisors who provide input and direction to AHS on numerous committees,
• supporters of our volunteer retail operations - where the proceeds are returned to AHS to provide patient care and comfort.

Vulnerable Persons PACT Community Treatment Order Case Management (VP PACT CTO CM)

VP PACT-CTO Case Management Team partners an AHS clinician (RN/RPN) with a CPS officer to manage identified clients who are under a Community Treatment Order. These teams utilize an assertive and intensive case management approach, and offender management strategy that combines crime reduction, enforcement strategies, and mental health treatment. This is done through effective engagement, supervision, and support while balancing the protection of public with the treatment of the client.

Wound Clinic - Integrated Home Care Program

Services are provided for the following:

• diabetes (diabetic) - acute wound management with follow-up
• care planning with consultation for chronic complex diabetic wounds
• gait and footwear for clients who experience a wound as a result of neuropathy and walking
• gait assessment for clients with associated plantar foot wounds
• biomechanical assessment to determine pressure sources for wound
  
• referral with assessment for custom made footwear according to Alberta Aids to Daily Living (AADL) guidelines for those clients with associated wounds
• wound care and prevention
• multi-disciplinary team - assess, manages and treats acute and complex chronic wounds
• assume care for client with acute and/or complex chronic wounds in partnership with physicians and surgeons or on a consultative basis
• Adjunctive Therapies such as Negative Pressure Wound Therapy (NPWT), Ultrasound, E-Stim and Total Contact Casting (TCC) for the alternative means of treating wounds
• provide drop-in wound care support for homeless and / or marginalized clients
