How do I get services in Edmonton Zone?

In the Edmonton Zone, you have many options for where to get palliative and end of life care:

  • your home
  • a hospital
  • a continuing care centre
  • a hospice

You need a referral from a health care professional to get palliative care services in hospitals. Tell your health care team that you would like a referral to the in-hospital Palliative Care Consult Service.

Palliative Services in Long Term Care and Supported Living Facilities

You need a referral from a health care professional to get palliative care services in long term care and supported living facilities. Tell your health care team that you would like a referral to the Palliative Care Consult Service.

Hospice Care

There is a hospice in Grande Prairie called Points West Living. You need a referral to use this service.

Psychological Services at the Central Alberta Cancer Centre or Red Deer Regional Hospital Centre

People dealing with end of life issues, grief, and loss of a loved one can get support and counselling. Call 403-343-4832.

Hospice Care

The Red Deer Hospice Society gives physical, social, emotional, and spiritual care in a community-based, home-like setting for people who are dying and for their loved ones.

How do I get services in the Calgary Zone?

In the Calgary Zone, you have many options for where to get palliative and end of life care and bereavement services.

Palliative Care Consult Service

Palliative Care Consult Teams are available to give expert advice for complex issues in all settings of care in the Calgary Zone. Doctor and nurse specialists, and sometimes other health care professionals such as social workers, are members of these teams.

In the City of Calgary, there is a Palliative Care Consult Team that serves:

  • each hospital
  • Tom Baker Cancer Center
  • Home Care
  • Continuing Care and Supportive Living sites

Outside the City of Calgary, there is a Rural Palliative Care Consult Team that serves patients, families, and staff in all care settings within the rural areas. In addition, at each Calgary Zone hospice there is a palliative care doctor who provides consultation as needed.

You need a referral from a health care professional to be seen by a Palliative Care Consult Team. You can tell your health care team you would like a referral to the Palliative Care Consult Service. They will make this referral for you if needed.

Palliative Home Care

  • If you live in Calgary: Ask your family doctor or health care provider to refer you to general home care. Or you can call 403-943-1920 to refer yourself for general home care. Your home care team will refer you to palliative home care and/or the Palliative Care Consult Service (urban) if needed.
  • If you live outside of Calgary: Ask your family doctor or health care provider to refer you to home care. Your doctor or the home care team (with your doctor’s approval) will refer you to the Rural Palliative Care Consult Service if needed.

Hospice

The Calgary Zone has 7 community residential hospices. They provide palliative care 24/7 in a home-like setting to people with life-limiting illnesses. The goals of care are focused on comfort and quality of life.

Tell your health care provider you would like to know more about hospice or ask for a Calgary Zone hospice brochure. You may also call the Palliative Care Consult Service at 403-944-2304.

Grief Support

The AHS Calgary Zone Grief Support Program offers grief counselling to adults 18 years and older who are grieving the death of someone in their life. The program offers individual or group education sessions. You can call the program at 403-955-8011.

Ask your family doctor to fill out a referral for general home care​ that may include palliative home care. Or you can call 403-388-6380 or (toll free) 1- 866-388-6380 to learn more.

The Symptom Control and Palliative Care Community Liaison Program helps to arrange palliative care services for people in the South Zone. Ask your family doctor to refer you.
