Add a child's card (under 16)

You can request Mobile Health Cards for your children by verifying your relationship.

To add a child's health card to your wallet:

  1. Sign in → select “Children's Mobile Health Card” on your dashboard.
  2. Select Add a child's Mobile Health Card.
  3. Enter your child's information, affirm the declaration, and submit.

If approved, the card is issued immediately to your Alberta Wallet. You may be asked for documentation to confirm your relationship.

If you can't be verified automatically:
You will see: "Unfortunately, we were unable to validate the information you submitted."

This usually means your family information needs updating, your child isn't linked to your AHCIP account, or custody/guardianship needs confirming.

You can upload further documentation to verify your relationship. If you do not have the required documents, contact us for help at 1-844-743-1905.

To resolve it:

  1. Gather your supporting documents: long-form birth certificate naming you as parent, adoption records, a custody/guardianship court order, a separation/divorce agreement showing parental responsibility, or other legal proof of relationship.
(Make sure your photo or scan is clear. Names, dates, and signatures must be legible.)
  2. Upload: select your relationship from the dropdown, add your phone number, and upload your file(s)
Accepted: .jpg, .jpeg, .png, .pdf — up to 10 MB each, 5 files max.
  3. Submit. You will get a confirmation code on screen and by email — keep it for your records.
  4. Wait for review. We will follow up by email with the outcome — this can take up to 4 weeks.
