Health summary

Appointments

The Appointments tool lets you keep track of your past and future appointments.

You can add information like who the appointment is with, the location, and other notes.

Family history

The Family History tool lets you track your family's health history. This helps your monitor health conditions early or stop you from developing a condition you are at risk for.

You can add family members, their health conditions, and other notes.

Immunizations

The Immunizations tool helps you keep track of all your immunizations given through public health services in Alberta, including COVID-19.

Select an immunization to view more information.

This may include immunizations given at a:

• pharmacy
• a doctor's office
• urgent care centre or an emergency department
• private travel clinic
• any place outside of Alberta

If you are age 14 years and older, you can print a copy of your immunization records from My Personal Records. If you do not have a MyHealth Alberta Account or you are missing immunizations, contact your local public health office, pharmacy, or family doctor for a copy of your immunizations.

Medical conditions

The Medical Conditions tool lets you keep track of conditions like asthma, diabetes, and high blood pressure.

If your condition changes or you no longer have a condition, you can edit or delete conditions at any time.

Medications

The Medications tool lets you keep track of your medication information. You will see two sections in the Medications tool. Medication records from Alberta Netcare are updated every day.

Dispensed Prescription Medications

This section shows your medications including:

• Medication information
• Prescription refill

When you start using My Personal Records, the Alberta Netcare Prescription Medications will show your medications for the last 18 months.

You cannot change or edit this information.

Other Medications

You can add other prescriptions, over-the-counter medications, and vitamin or herbal supplements.

You can enter the following information:

• Medication name
• Date the prescription was filled
• The day you started your over-the-counter medication, such as vitamin or herbal supplement
• Strength (such as 500 mg)
• Dosage (such as 1 tablet or 2 drops)
• Frequency (such as once a day)
• Route (for example by mouth or inhaler)
• Date you discontinued taking the medication

Referrals

The Referrals tool in My Personal Records lets you view information about some referrals submitted on or after April 26, 2025.

A referral is a request from your healthcare provider to another provider or specialist for advice, assessment, or treatment.

Referrals go through many stages before an appointment is scheduled. As your referral moves through the process, status updates may be available in My Personal Records.

If you do not see your referral in My Personal Records or MyChart, your healthcare provider may have sent it directly to the specialist instead. Referral information becomes available as soon as it is received.
