Planning for Your Discharge from the Hospital

You, your family, and your healthcare team will work together on a plan for your discharge from the hospital.

Being discharged brings new challenges and adjustments. Planning is important for your safety and comfort whether you are going back home, moving in with a family member, or going to a rehabilitation centre or care facility. You will also need to plan for any changes that will need to be made to your home for you to live safely.

There are different options for rehabilitation after an amputation. Some patients are able to go home and have rehabilitation as an outpatient. Others will go to a rehabilitation facility to get more inpatient rehabilitation before going home.

Before you leave the hospital, someone from the healthcare team will set up follow-up appointments with your family doctor, surgeon or physiatrist.
