Central Intake for referral to Calgary Zone Home Care, Supportive Living and Long Term Care.
Anyone living in Alberta with a valid provincial health care card (or registration in progress) can receive Home Care services as long as their health care needs can be safely met in the home. Consistent access criteria have been developed provincially for Supportive Living and Long Term Care and serve as general guidelines. Each client will be individually assessed.

Anyone can make a referral to Continuing Care provided the client is in agreement.

Access Mental Health is a non-urgent service providing information, consultation, and referrals for individuals with addiction and / or mental health concerns. Mental Health Clinicians complete a clinical interview over the telephone to assess the individuals’ needs. Mental Health Clinicians are familiar with both Alberta Health Services and community-based programs and will explore all options in order to refer individuals to the most appropriate resources.

This is the Central Triage Department for the Alberta Healthy Living Program (AHLP) in the Calgary Zone. Referrals that meet criteria will be processed and patient's will be scheduled into the referred sevice.
All patients must have at least one chronic condition to be eligible for our program.
The Alberta Healthy Living Program offers health education classes, self-management workshops, nutrition counselling for adults living in the community or living with a chronic health condition.
Adults living with a nutrition-related health condition are eligible for nutrition counselling with a registered dietitian. Participants will be asked to attend a relevant Health Education class, if available, before booking a nutrition counselling appointment.
Eligibility criteria differs between services.
This program works to make sure people with lung cancer get faster access to treatment through special rapid access clinics in Edmonton and Calgary. The program seeks to reshape cancer care in Alberta by improving the patient journey and outcomes and standardize care delivery across the province.

Following AADL eligibility criteria, AHS' Calgary Community Aids for Independent Living (CCAIL) team provides clinical assessment for the following basic medical equipment and supplies, when required fbecause of a long-term disability or chronic illness (6months or greater):
Please note: Clients who have recently sustained injury, received and or have a planned surgical intervention, or are undergoing active rehabilitation, are not AADL eligible for at least six months and should not be referred to CCAIL.
Referral sources should consider short-term equipment loans or rental options through other programs, until long term need has been demonstrated.

This is the Central Triage Department for Calgary COPD & Asthma Program. This program provides comprehensive respiratory education for patients with COPD, asthma, or smokers at risk. The session is one to one either by phone or Zoom and will take 45 minutes to 60 minutes to complete with a Certified Respiratory Educator. Spirometry is no longer provided.

This service provides a single point for service access and delivery approach that provides Albertans with reasonable, timely and appropriate access to publicly-funded continuing care health services. It is based on availability and determination of unmet need.
There are no fees for Professional or Personal Care Services in Home Care. Accommodation Rates are standardized in Supportive Living and Long Term Care Centres that are funded by Alberta Health Services.
CHPS (Community Health Promotion Services) provides evidence-based substance use prevention and mental health promotion services and resource in Calgary. Our focus is building protective factors and reducing risk factors associated with substance misuse and mental health disorders. All services are free and can be accessed by contacting the office.

The Community Helpers Program takes place in community-based settings (schools, workplaces, community organizations) and enhances community capacity for mental health through an assets based approach. Primarily, the Community Helpers Program targets youth and young adults between the ages of 12 - 30.
In every community there are people whom others naturally turn to for support. They are from all walks of life and are considered by others to be helpers. Helpers possess characteristics like empathy, care, and compassion. The Community Helpers Program identifies these helpers and offers training to strengthen their natural abilities. Helpers learn a variety of topics and skills including effective communication, self-care, coping with stress, knowing when to refer people to professional services, handling crisis situations, and suicide awareness. In addition to developing skills and increasing knowledge, Helpers are introduced to community services and professional supports. This connection bridges informal and formal supports in the community, enabling helpers to refer peers and family to expert support when needed.
Objectives of the Community Helpers Program include:
Youth and adults, ages 12 years and older.
In person and virtual asthma education appointments for children and their families with mild to moderate asthma that includes a customized Asthma Action Plan for every patient. A breathing exercise (simple spirometry) for patients 6+ years of age will be scheduled as required.
Children from 2+ years of age. Patients 6+ years of age may also have a breathing exercise (simple spirometry) as required.
Not available for evening, weekend or holiday appointments. Patients should also be prepared to travel anywhere in Calgary for an acute consultation appointment.
Coordination of a single appointment 5 - 7 days after a pediatric emergency, urgent care or inpatient discharge when the family’s usual physician is not available for a short notice, acute follow-up appointment.
Patients should return to an emergency department if they have any concerns while they are waiting for an appoimtment.

The Carewest Comprehensive Community Care (C3) Program operates within two Calgary locations: Beddington C3 in northwest Calgary, and Sarcee C3 in the southwest. The C3 program is uniquely modelled to support vulnerable, older persons residing in the community by providing timely, comprehensive medical care that is integrated seamlessly across a wide spectrum of services. In addition to the emotional, social, and recreational support provided by a traditional adult day program, C3's services also includes the provision of home care services, social work, occupational and physiotherapy, mental health support, and primary care provided by a family physician with additional training and certification in care of the elderly. Nursing and physician telephone support (via the on-call nurse) is available 24 / 7, 365 days a year. Clients who experience acute medical decompensation can be managed effectively at home, in the clinic, or our overnight treatment bed located at our Sarcee location, potentially avoiding unnecessary visits to emergency departments and hospital.

Medications costs are determined by our pharmacy and will be processed monthly.
Subsidy may be available to help cover the monthly fee for the C3 program based on eligibility. Coverage for medications will be dependent on Alberta Blue Cross and any additional benefits the client may have.

Once triaged, referrals are allocated to EPA resources in the zone. Referrals that meet criteria have a high degree of suspicion for incomplete miscarriage (under 12 weeks for FMC EPA or up to 20 weeks for SHC EPA), or pregnancy of unknown viability. We do not manage pregnancy of unknown location (ectopic pregnancy). Patients who meet the referral criteria are then referred to EPA clinics for management options (which include expectant management, medical or surgical management).
High degree of suspicion of incomplete miscarriage (under 12 weeks of gestation for FMC EPA and under 20 weeks gestation for SHC EPA).
Nurses are there 24 / 7 to offer support, information, and referrals to new mothers and their families for the first 2 months after their baby’s birth.

The Eating Disorder Program (CEDP) provides specialized services to meet the needs of Albertans of all ages with a diagnosis of anorexia nervosa, bulimia nervosa, and other specified feeding or eating disorders. Services include:

The Hepatology service sees patients for a variety of liver conditions, including cirrhosis, chronic liver disease, some liver malignancies, fatty liver disease, and Viral Hepatitis (B and C). We offer the following services:
Patients requiring surgical interventions should be referred to the appropriate surgical service. Patients requiring cancer treatments should be referred to the appropriate Oncology service.

As part of the Integrated Home Care Program, provides care and support to meet the assessed needs of the client after normal business hours. Works with the client, their family, other professional health care staff and partners to help clients be as independent as possible.
You must be a client of the Home Care Program to be able to access the after hours Home Care Response Team.

This service aims to provide integrated healthcare for acutely ill patients by providing services in the patients home that would be otherwise delivered in a hospital or outpatient clinic within 3 hours of receiving a referral.
Response time is confirmed upon receipt of referral based upon patient acuity or when service is requested.
Upon receipt of referral a community paramedic will be dispatched to the patients dwelling where they will conduct a comprehensive assessment and develop a customized care plan after consultation with the patient’s most responsible healthcare prescriber.
A detailed record of the patient encounter will be sent to the patients’ healthcare team.

The Pediatric and Adolescent Gynecology Clinic offers timely, age-appropriate gynecological care for children and adolescents in a family-centered facilities. Pediatric Rehabilitation services support the development of children and youth to participate in meaningful activities of daily life at home and in the community. Focus is on the early years and early intervention to prevent health complications and improve long term health and wellbeing. Services support the development of children and youth to participate in meaningful activities of daily life at home and in the community. We want to understand what's important to your child and family. Services are goal-driven; starting and continuing services is based on having functional reasons / goals for rehabilitation. Therapists will work with parents to provide the tools and strategies to apply in the context of their home and life. Therapy may look like 'play', depending on your child's age. We will work together so you and your child are supported to move to the next developmental stage or between providers and services. Services are provided by phone, zoom and some in person sessions as determined by your therapist. They may be in a group or one to one. Services provided are related to (neuro) developmental conditions and / or medical conditions. Social Work and Registered Dietitian services are available to support rehabilitation outcomes.

This service provides doctors with access to mental health consultants (psychiatric registered nurses, social workers, psychologists, psychiatrists) who take part in a client’s regular appointments. Mental health consultants can help doctors with:
Doctors who are part of a primary care network can also refer clients with mental or behavioural health concerns to behavioural health consultants. These consultants work in primary care clinics and can make appointments with patients that don’t include the doctor and are shorter than shared care sessions.

Our clinic is an interdisciplinary lead clinic providing support to patients experiencing persistent symptoms caused by Covid-19 and are at least 12 weeks post the acute phase of the infection. Inter-professional Outpatient Programs (IPOP) have been established to address COVID symptoms in patients where their symptoms last longer than 12 weeks. The IPOP clinics will provide medical and / or rehabilitation supports and services based on each patient’s needs by connecting primary care, specialty medicine, and allied health professionals. The IPOP teams are comprised of nurse practitioners (NPs), physicians, registered nurses (RNs), and Allied Health professionals who will develop/collaborate on and/or oversee patients’ care plans. People with long COVID report experiencing different combinations of the following symptoms: fatigue, difficulty thinking (“brain fog”), cognitive changes, headache, loss of smell / taste, dizziness, palpitations, chest pain, dyspnea, ongoing cough, joint / muscle pain, fever, depression, and anxiety. Symptoms often get worse after physical or mental activity.

The Urgent Neurology Clinic provides urgent access and assessment service for patients with new or rapidly progressing neurological symptoms, which can or should no longer be managed by a patient's primary care team.
