Immunized in Alberta

If you have been immunized recently in Alberta through a public health clinic, pharmacy, or doctor’s office, you do not need to upload any immunization records.

All immunizations given in Alberta will be added to your MyHealth Records account within 2 to 3 weeks.

• If any routine immunizations you received in Alberta are not showing in your MyHealth Records account, contact your immunization provider or contact MyHealth Records Support.
• For missing COVID-19 immunization records, contact the COVID Records Helpdesk.

Submitting an immunization record

Only submit immunization records if you or your child meets 1 or more of the following criteria and lives in Alberta:

• The child is in Grade 9 or younger.
• You have an appointment with public health.
• You are submitting records for COVID-19 immunization or measles immunization that were given outside of Alberta or Canada.
• The immunization records have been requested by public health.

You must upload your official immunization records. The uploaded records must show a first name, last name, date of birth, vaccine name, and date of immunization.

For COVID-19 immunization records, please submit your digital record including the QR code if available.
