Preventative Health Testing Services Claim

Eligibility

You may be eligible to submit online if you:

• Are a resident of Alberta
• Are 18 years of age and older
• Have a verified MyHealth Alberta Account

Parents and guardians can submit a claim for reimbursement online on behalf of dependents younger than 18 years of age if parent/guardian access has been established. If a parent/guardian submits an application for a dependent, all communication and any eligible reimbursement payments will be sent to the applicant.

If parent/guardian access has not been established, submission for a dependent under 18 years of age can be completed through an alternative submission option.

Submitting a claim

Follow the steps below to submit a claim online:

1. Gather required documentation.
2. Login to your MyHealth Alberta Account and navigate to the Services page. Once on the Services page, select Preventative Health Testing Services Claim.
3. Make sure you meet the program requirements. Information on eligibility can be found here.
4. Select the name of the person the reimbursement request is for. This may be yourself or a dependent.


If parent/guardian access is set up, parents can select a dependent from the drop-down list.

If not, dependents must be submitted through an alternative method. More information on the program and alternative submission methods is available here.

5. Confirm that the mailing address is correct. If the address displayed is incorrect, please visit a registries office and update your address before completing the online submission.
6. Select the type(s) of tests you will submit as part of the claim and provide your phone number. Submissions can include up to 10 documents, with a size limit of 100 MB per document. You must upload a minimum of 2 documents.
7. Confirm the declaration and click Continue.

You will see a confirmation screen and reference number for your submission. An email confirmation will also be sent to your email address associated with your MyHealth Alberta Account.
