Your immunization record is a list of all of the vaccines you have had, and when you had them. In Alberta, you can get your immunization records online using your MyHealth Records account. It can take 2 to 3 weeks after getting your vaccines for the records to be available in MyHealth Records.
If you don’t have a MyHealth Records account, you can register for one if you are age 14 years and older. If you are under age 14 years, you can get your records from an Alberta Health Services public health or community health centre.
Note: Some of the vaccines you have had may not be on your immunization record. These could include vaccines you had outside of Alberta or from a(n):
If you received an immunization outside of Alberta, you can upload the record to be reviewed, verified, and added to your MyHealth Records account.
You can submit records for yourself or for a person under age 18 years if you are their parent or legal guardian.
If you don't have a record of the immunization, try contacting the healthcare provider or regional health authority where you were immunized.
If you are age 14 years and older, you can get your immunization records online using your MyHealth Records account.

Immunization records are available from your local Alberta Health Services public health or community health centre.
To request records for anyone under age 18 years, you must be their legal guardian. You may be asked to show proof that you are the guardian.
If you are age 18 years or older, you must request your records yourself.
To find the public health or community health centre nearest you, go to the Community Health Centres search tool and enter your location or postal code.
For COVID-19 immunization records, use the Alberta COVID-19 records tool to get your official Alberta-issued COVID-19 vaccine record with QR code.
All COVID-19 immunizations given through an Alberta Health Services clinic, an Alberta pharmacy, or Alberta doctor’s office will be automatically included.
It can take 2 to 3 weeks after getting your vaccine for the record to be available in the COVID-19 records tool and MyHealth Records.
