Account setup & settings

You are eligible if you are:

• Aged 16 or older
• A Northwest Territories resident with a valid ID card
• Receiving care at an Alberta hospital or ambulatory care facility

You will need:

• A valid government-issued ID with your current address
• Your Northwest Territories Personal Health Number
• A recent visit to an Alberta hospital or ambulatory care facility
• Completed document verification (valid for up to one year or until your ID expires)

1. Create an Alberta.ca account
Select "Create account" and sign up using your email address on Alberta.ca.

2. Verify your account
Select "Verify your Alberta.ca account via this link," then select "Verify account" to start document verification.

3. Verify your documents
Verify your documents through the Interac document verification service. You will be prompted to complete this verification on your mobile device.

Note: Northwest Territories residents MUST use identification that displays their Northwest Territories address.

4. Enter your health number
Enter your Northwest Territories Health Care Card personal health number to finish setup — your ID must show your Northwest Territories address.

You are set up!

Your health records are now available from the home page of your MyHealth Alberta Account.
